I think it's likely she and Kovu are twins, Vitani being the older one. Even if they are not twins, I don't think Scar is the father of any of them though, judging by the way they speak about him in the film.

It says in the movie that Vitani and Nuka are Scar's cubs, where as Kovu is an unknown lion's. All three have the same mother though. Making Kovu Vitani and Nuka's half brother.
